Click the Contact button on a provider’s information card to open a Contact Provider Form.
You can fill out the form to request a follow-up from the provider. We recommend including your detailed question or request in the Enquiry/Message field. When you click Submit, the provider will receive a notification with all of the details shared in the form in their Provider account and by email.
From there, the provider can accept your request or re-assign you to another contact in their Provider account. A contact from the provider’s side can reach out to you regarding your request using the contact information you provided in the Contact Provider Form.
